You will need:
Minor patients are required to have at least one caregiver assigned to their accounts at all times. Only parents or legal guardians 21 years of age and older are eligible to serve as caregivers for minor patients.
Any Maryland resident whose provider approves medical cannabis as a treatment option for a qualifying medical condition is eligible to register with the MMCC as a patient. Qualifying medical conditions include cachexia, anorexia, wasting syndrome, chronic or severe pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, and post-traumatic stress disorder. However, the provider may determine that other conditions may qualify provided that they are severe, other medical treatment have been ineffective, and medical cannabis is likely to help.
Please call or request appointment online to see our physician to be evaluated for the certification. It is your responsibility to bring with you to the appointment all your prior medical documentation and records which may help our physician to determine whether you suffer from one of the qualifying conditions and whether medical cannabis would be appropriate for you. In order for your medical condition to qualify for medical cannabis, our physician must determine that its severe, that other medical treatments must have proven ineffective for the your condition, and that the symptoms must reasonably be expected to be relieved by the use of medical cannabis.
Once the evaluation is completed our physician will need your MMCC-issued Patient ID Number to issue the certification for medical cannabis through the Commission’s secure online application.
After registering, obtaining a valid written certification from a registered provider, and printing a temporary MMCC Patient ID card, patients are able to visit licensed Maryland dispensaries to purchase medical cannabis.
The review of the patients’ medical condition, previous diagnostic and therapeutic interventions and determination whether the medical cannabis would be a useful and appropriate treatment option for the patient. If the determination is favorable, written certification for medical cannabis treatment is provided to the patient.
Continued care to optimize the patient’s treatment with the focus on the medical cannabis as the therapeutic modality.
